Our personalised NDIS support services
Unity Path Care offers a comprehensive range of NDIS support services tailored to your individual needs and goals. We focus on empowering participants to live more independently, engage with their community, and achieve a higher quality of life.
Assistance with daily living.
We support participants with everyday activities to help them live independently, safely, and comfortably in their own home. Our support is person-centred and tailored to each participant’s NDIS plan, goals, and individual needs.
Our goal is to empower participants to maintain their independence while receiving the right level of support in a respectful and dignified way.
Community Access & Social Support
We support participants to engage with their local community, build social connections, and participate in activities they enjoy. Community access supports are tailored to the participant’s interests, abilities, and NDIS goals.
This may include attending appointments, social outings, community events and recreational activities. Our focus is to help participants develop confidence, independence, and meaningful relationships within their community.
Transport & in-home support
Our trained and compassionate support workers provide one-on-one support in the comfort and safety of your home. We work closely with participants, families, and support coordinators to tailor support services to each participant’s NDIS plan and personal goals.
Support may include personal care, mobility assistance, social support help with daily tasks, and supervision where required. We aim to create a safe, supportive, and positive environment where participants feel comfortable and respected.
We provide transport support to help participants attend appointments, work, school, shopping, and community activities. All transport supports are delivered in line with the participant’s NDIS plan and individual needs.

Building independence & community connection
We support participants by building daily living skills like personal care, cooking, and household tasks, enabling control over their lives. We also encourage community connection, helping access shopping, appointments, social outings, and activities to foster engagement and confidence.

Unity Path Care provides reliable transport, ensuring you reach medical appointments and community activities safely. Our consistent in-home support offers stability through trusted workers. We also prioritise emotional wellbeing with companionship, encouragement, and respectful care, all within personalised support plans aligned with your NDIS goals.
